What is the TruStar Method

Our modern investigation framework: the five Stars, an AI bias audit, and verified closed loop actions.

The TruStar Method is a modern incident investigation framework built to address what experienced safety practitioners have long complained about in older root cause tools. It runs on every TruStar Connect investigation by default, and is supported by an independent AI bias audit on every analysis.

The five Stars

1. People
Actions, decisions, awareness, fatigue
2. Conditions
Environment, equipment, hazards, weather
3. Systems
Procedures, JSAs, checklists, training
4. Leadership
Supervision, communication, schedule pressure
5. Defenses
Barriers, controls, safeguards
Every analysis considers all five Stars, not just the people involved.
  • People: why a reasonable, competent person did what made sense to them in the moment. This is the start of the analysis, not the end.
  • Conditions: the physical and environmental reality, including equipment, layout, weather, lighting, and ergonomics.
  • Systems: the procedures, training, JSAs, and permits meant to guide the work, and the gap between what the documents say and what people actually do.
  • Leadership: the organizational signals about what really matters, including schedule pressure, resourcing, and how past concerns were received. This is the star most often missing from traditional investigations.
  • Defenses: the barriers that were supposed to catch the problem and did not, inspired by the Swiss cheese model of layered protection.

What makes it different

  • Evidence first, framework second: the AI extracts the facts before offering any causal hypothesis, so the analysis is not biased by a category chosen too early.
  • Organizational and leadership factors are weighed equally, not treated as an afterthought to individual error.
  • An independent bias audit runs on every analysis, checking for hindsight bias, individual blame framing, and premature closure, which you must read before sign off.
  • Corrective actions are ranked by the hierarchy of controls, and the system flags action plans that lean on weaker controls or leave a causal factor unaddressed.
  • Effectiveness is verified on the schedule your organization sets, because completing an action is not the same as proving it worked.
  • No multi day certification is required. The AI handles the methodology so you can be productive on day one.
When it fits

The TruStar Method suits the full range of workplace incidents, from a strain injury to a serious process safety event, especially where organizational and leadership factors matter. For teams committed to a specific legacy framework, SCAT, ICAM, HFACS, 5 Whys, and Bowtie are all available too.
