Effectiveness Reviews
Confirm a fix actually worked, not just that it was done.
Marking a corrective action complete is not the end of the story. An effectiveness review checks back after the fact to confirm the fix held and the problem has not returned.
- Schedule a review for a completed corrective action.
- Record whether the fix is still working in practice.
- Reopen or escalate if the issue has resurfaced.
Opening a review prepares a short list of questions written for that particular action. They come from whatever the action was raised for: the causal factor when it came out of an investigation, or the deficiency on the form when a workflow rule raised it from an inspection. The questions are saved with the review, so you and an auditor see the same ones later.
Demonstrating that fixes are verified and sustained, not just logged, is a hallmark of a mature safety program and a strong COR audit.
