Hazard Register: identify, assess, and control hazards

Build org-wide and project hazard registers with 5x5 risk and the hierarchy of controls.

The Hazard Register is your master list of the hazards your work creates, how serious each one is, and the controls that keep it in check. It is a core part of a COR hazard assessment, alongside the field-level FLHA forms your crews fill out day to day.

Org-wide or project registers

Keep an org-wide register for hazards that apply across the company, and project-specific registers for hazards unique to a job. Switch between them with the selector at the top of the page. New hazards default to whichever register you are viewing.

Assess risk on the 5x5 matrix

  • Rate each hazard for likelihood and severity from 1 to 5, the same matrix used in investigations. The score and its risk band, from Low to Extreme, are color coded.
  • Not sure what a rating means? Use the Risk Matrix Guide button at the top of the page to open a reference of the severity levels, likelihood scale, and rating bands.
  • Record both an initial risk, before controls, and a residual risk, the risk that remains once controls are in place. The gap shows how much your controls reduce the risk.

Record controls by hierarchy

Add the controls for each hazard from most to least effective: elimination, substitution, engineering, administrative, then PPE. A control can link to the document that backs it, such as a safe work practice.

Build it with AI

  • The register is split into collapsible sections: an org-wide register and one per project. Each section has its own Build with AI and New hazard buttons.
  • Building a project register asks for that project scope of work, then drafts hazards from the scope combined with your business profile. The org-wide build uses the business profile on its own.
  • AI-built hazards arrive with a description, an initial and a residual risk rating (assuming the recommended controls are in place), you set as the owner, and a review date one year out. Everything is editable.
  • Suggest controls drafts controls for a single hazard. You can edit any control, Auto-link connects controls to existing documents, and Create with AI drafts a missing one. Each control can link to several documents.
AI is a starting point

These suggestions are guidance to speed you up, not a replacement for judgment. Review every hazard, edit anything that does not fit, and confirm the ratings and controls with the people doing the work.

Review reminders

When a hazard you own is within 30 days of its review date, it appears as a task on your Tasks page so nothing slips.

Export to Excel

  • Each section has an Export button that downloads that register as a branded, landscape Excel workbook: activity, hazard, description, initial and residual risk, controls by hierarchy, owner, review date, and linked documents. It is the traditional hazard assessment format, ready to print or hand to a client.

Import an existing assessment

  • Use the Import button on a section to upload an existing hazard assessment as an Excel file or PDF. The assistant reads it for activities, hazards, controls, and risk rankings, maps them into the app format, and assigns sensible initial and residual ratings where the document has none.
  • You review everything in the usual confirmation window before adding. Any hazard whose title already exists in that register is marked and skipped, so you do not create duplicates.

Connect field assessments to the register

  • Forms can include a "Task Hazard Selector" field. On a Field-Level Hazard Assessment, the worker describes the task and generates a short, task specific list of likely hazards, each with suggested controls. It is meant to be focused on the job at hand, not a full copy of the register.
  • Every suggested hazard and control is editable. The crew keeps the ones that apply, adjusts the wording, checks which controls are in place, and adds anything the suggestions missed. The default FLHA template already uses this field.
  • Each hazard can be classified by its energy source from the energy wheel: gravity, motion, mechanical, electrical, pressure, temperature, chemical, biological, radiation, or sound. The AI suggestions come pre-classified, and a small wheel shows at a glance which energies the task involves. Energy-based hazard recognition helps a crew see what could actually hurt them, and the classification carries through to the submitted record and the PDF.
  • A submitted FLHA can be edited through the day as conditions change. Open the assessment and tap Edit to update hazards, controls, PPE, and the rest, then save. Changes are kept on the same record rather than starting a new form, and every edit is written to a Revision History on the assessment and its PDF, showing who changed what and when. Editing is limited to the worker who submitted the FLHA or a manager, and the work-area photo from the original submission is preserved.
  • Any hazard on the assessment can be flagged to suggest for the Hazard Register. When you review the submission, a "Hazards Flagged For The Register" panel lets you promote each one, with its controls, into the register in one tap. This keeps the register the single source of truth while letting the field feed it.
  • Your registers are also listed in the Documents area, inside the Hazard Assessments folder, beside your field-level assessments. Each one links straight into the register, so everything an auditor looks for sits in one place.
