Per-worker sign-off on the FLHA

Added

New "Worker Sign-off" field. It shows a signature box for each worker selected in a Workers field on the form, so everyone on the task signs before the assessment is submitted. Extra signers who are not in the system can be added on site.
The recommended FLHA now uses per-worker sign-off in place of the single signature line.

Fixed

The recommended FLHA asked for the project twice. The duplicate Project field is removed, since the project is already chosen at the top of every form.

Leaner FLHA, weather on site, and recommended forms

Changed

Streamlined the recommended FLHA. The separate Task Description and Controls fields are gone because the Task Hazard Selector already captures the task and recommends controls. The Date and Location fields are gone too: each submission is automatically date and time stamped, and the project is already chosen at the top.

Added

Weather on site. When a form is submitted with location captured, the current conditions and the day's forecast (temperature, wind, and chance of precipitation) are recorded on the submission and shown on screen and in the PDF.
PPE and other multi-select questions can now include an "Other" option that reveals a text box, so crews can add an item that is not on the list. Turn it on per field in the form builder.
A "Recommended Forms" button on the Inspections and Forms page creates the recommended COR forms (Safety Meeting, Toolbox Talk, FLHA). It recreates any that were deleted, with the current recommended fields, and leaves your existing forms untouched.

Fix form editor crash on older templates

Fixed

Editing a form that still used the previous "Hazards From Register" field could crash with a client-side error. That field now migrates automatically to the new Task Hazard Selector wherever a form is opened, filled, or viewed, so no manual swap is needed.
Hardened the form builder so an unrecognized field type can no longer crash the editor.

Task-aware hazard selector for field assessments

Changed

Replaced the "Hazards From Register" field with a "Task Hazard Selector". The old field pulled every hazard for the project, which was too long for a Field-Level Hazard Assessment. The new field is task focused.

Added

On an FLHA, a worker describes the task and generates a concise list of likely hazards, each with suggested controls, in a single tap. The list is meant to be short and specific to the job at hand, not a full register.
Every suggested hazard and control is editable. Workers keep the ones that apply, edit the wording, check off which controls are in place, and add their own hazards.
Any hazard can be flagged to suggest for the Hazard Register. Managers reviewing the submission can promote those into the register in one tap, exactly as before.

Field assessments connect to the Hazard Register

Added

New "Hazards From Register" field for forms. On a Field-Level Hazard Assessment, workers can pull the active hazards for their project (plus org-wide hazards) straight from the Hazard Register, tick the ones that apply, and confirm each control is in place.
Workers can add a hazard that is not in the register and flag it to be added later, so nothing identified on site gets lost.
When reviewing a submission, managers see a "Hazards Flagged For The Register" panel and can add each flagged hazard, with its controls, to the Hazard Register in one tap.

Improved

Submission views and PDFs now show selected hazards with each control marked in place or not, plus any hazards added on site.
The default FLHA template now uses the register-connected hazards field in place of the free-text Hazards and Controls boxes.

No hazards lost on import

Fixed

A very dense batch of rows could still exceed the assistant output limit and be dropped, so a few hazards went missing from an otherwise successful import. A batch that hits the limit is now split in half and retried, down to single rows, so every readable hazard comes through.

Import handles dense assessments

Fixed

Importing a detailed assessment could still fail when rows held long hazard and control lists, because each batch generated more than the assistant could return at once. Batches are now smaller, the per-batch limit is higher, and each source row maps to one hazard by default, so dense documents import successfully.

Added

Import now shows a notice while the document is analyzed, letting you know it can take a few minutes for a large assessment and to keep the tab open.

Reliable import for large assessments

Fixed

Importing a large hazard assessment from Excel could fail because the whole document was read in one pass and exceeded the assistant output limit. The spreadsheet is now read in smaller batches and the results merged, so large registers import reliably.

Improved

Import now skips instruction, title, risk-matrix, and sign-off sheets so it focuses on the actual hazard rows, and a single unreadable batch no longer fails the whole import.

Import hazards from an existing assessment

Added

Each register section now has an Import button. Upload an existing hazard assessment as an Excel file or PDF and the assistant scans it for activities, hazards, controls, and risk rankings, then maps them into the app format.
Where the source document has no initial or residual rating, the assistant assigns a sensible one using its judgment, which you can edit.
Imported hazards are shown in the usual confirmation window for review. Any hazard already in that register is marked and skipped, so duplicates are not created.

Export the Hazard Register to Excel

Added

Each register section now has an Export button that downloads a branded, landscape Excel workbook in the traditional hazard assessment format: activity, hazard, description, initial and residual risk (colored by band), controls by hierarchy, owner, review date, and linked documents.

Fixed

Build with AI could fail to return any hazards on an org-wide register because the longer hazard list ran past the assistant output limit. The limit has been raised so full registers generate reliably.

Hazard Register polish

Added

You can now delete a hazard directly from the register list, with a confirmation prompt before anything is removed.

Fixed

Control cards no longer squish the description into a narrow column on phones. The control type sits on its own line above the full-width text.
The bottom navigation bar now stays locked to the bottom of the screen on mobile instead of drifting up while scrolling.

Hazard Register, reorganized

Added

The Hazard Register is now organized into collapsible sections: an org-wide register and one for each project, each with its own Build with AI and New hazard buttons.
Building a project register asks for that project scope of work and drafts hazards from the scope combined with your business profile.
AI now fills in a description and a residual risk rating for each hazard, sets you as the owner, and sets a review date one year out. You can edit all of it.
A review reminder now appears on your Tasks page when a hazard you own is within 30 days of its review date.
You can now edit a control inline, and link more than one document to a control.

Improved

A collapsible info box explains that AI suggestions are guidance to be verified and edited by a person.
The document linking controls on a hazard now stack cleanly on mobile and no longer cause sideways scrolling.

AI for the Hazard Register

Added

Build with AI on the Hazard Register. It suggests a full list of hazards for your business, each with an initial risk rating and a set of controls, which you review and add to an org-wide or project register.
Suggest controls drafts controls for any single hazard by the hierarchy of controls, ready to review and add.
Controls now link to documents. Auto-link connects a hazard’s controls to your existing safe work practices and procedures, and each control has a picker to link any document by hand.
When a control has no document, Create with AI drafts a safe work practice in your Documents, links it to the control, and opens it for review.

Hazard Register

Added

A new Hazard Register under Safety. Build an org-wide register for company-wide hazards and project-specific registers for individual jobs, switching between them with one selector.
Assess each hazard on the same 5x5 risk matrix used in investigations, with both an initial risk and a residual risk after controls, color coded from Low to Extreme.
Record the controls for each hazard ordered by the hierarchy of controls: elimination, substitution, engineering, administrative, then PPE. Owners, admins, and managers can manage the register.

Knowledge Base refresh and clearer Create menu

The mobile Create menu now says New Form Submission instead of New Inspection, since not every form is an inspection.

Cleaner document list on every screen

Improved

The document list now lays out properly on phones and desktop. On a phone the action buttons sit on their own row beneath the title instead of crowding it, so long titles are easy to read.

Documents in the list now show their status, including an Approved pill, along with the revision number and the date that revision was made.

Lock sign-off on empty documents

Fixed

A brand new, empty document showed an active approval form. Sign-off is now locked, greyed out, and labelled with a clear message whenever the document has no saved content to approve, in addition to the unsaved-changes case.

Save before you sign

Fixed

A document could be signed and approved before its latest edits were saved, which approved the stored version and discarded unsaved content, including text drafted with AI. The Sign and approve and Assign controls are now locked until the document is saved, then unlock automatically.

A short, collapsible explanation in the sign-off panel describing why a document has to be saved before it can be approved.

Document sign-off with digital signatures

Added

Approval sign-off for documents. A document must now be signed off by its creator, or by a selected manager, admin, or owner, before it can be approved. Sign with a real digital signature drawn right in the app, and it appears on the document and on the PDF.
Assign a document to a manager, admin, or owner for approval. It shows up in their Tasks under "Documents to sign" so they know it is waiting on them.

Changed

Approved status is now reached by signing the document, not by setting it directly. Editing an approved document still creates a new revision and requires a fresh signature.

Fixed

A folder document count now decreases correctly after you delete a document.

Draft your safety documents with AI

Added

Business profile in Organization settings: describe what your business does and the industries you work in, and set your country and (for Canada) the provinces and territories you operate in. The document assistant uses this to draft relevant, jurisdiction-specific content.
Write with AI in the document editor. In the Health and Safety Policy folder it drafts your single overarching policy, complete with a senior manager signature block and a note that it must be re-signed annually to stay COR compliant. In Safe Work Practice, Safe Job Procedure, and Code of Practice folders it asks what you want to write and suggests topics based on your business.
AI drafts are tailored to the OHS legislation of the jurisdictions you set, and are fully editable so you can use them as a starting point.

Write controlled documents in the app

Added

You can now author documents directly in a Documents folder. New document opens an editor with a title, status, and a simple writing area that supports headings and bullets, so you can write your policies, safe work practices, and procedures in the app.
Every document has a status (draft, in review, approved, archived) and is revision tracked. Editing an approved document automatically saves the previous version to its history, bumps the revision, and returns it to draft for re-approval.
Download any authored document as a branded PDF, with your organization logo, document number, revision, and status.

COR category selector in the form builder

Added

The form builder now has a COR category selector, so you can set whether a form is an inspection, hazard assessment, safety meeting, toolbox talk, or other category. Submissions then file automatically into the matching Documents folder.
Scanned forms and AI generated forms now arrive with a suggested COR category already filled in, based on the form type, so you just confirm it.

Added

The Inspections, Meeting Minutes, and Hazard Assessments folders now show live form submissions: inspections, safety meetings and toolbox talks, and field-level hazard assessments, each linking to the full submission with a one click PDF.
Add COR starter folders now also creates three ready to use forms: Safety Meeting Minutes, Toolbox Talk, and Field-Level Hazard Assessment (FLHA). Every time one is submitted it appears automatically in the matching folder.

Changed

Your existing form templates are automatically sorted into the right folder by name and type, so inspections and hazard assessments you already use show up without any setup.

Expandable training rows and a fixed investigation link

In the Training and Orientation folder, click a worker to expand their record and see every required course with its status: valid, expiring, expired, or missing, each with its expiry date, plus any additional certifications they hold.

Linked folders: live records in Documents

Added

Documents folders can now show live records from other parts of the app instead of stored files. The Incident Reports folder shows your investigations, and the Training and Orientation folder shows your training matrix, each always current with nothing to file by hand.
Linked folders are marked Live records in the folder list, and each record links straight to the full entry, with a one click PDF for investigations.

Changed

Add COR starter folders now also upgrades your existing Incident Reports and Training and Orientation folders to linked, so you can turn this on without recreating anything.

Controlled documents and COR starter folders

Added

One click to add a set of COR-aligned starter folders to Documents: Health and Safety Policy, Roles and Responsibilities, Safe Work Practices, Safe Job Procedures, Codes of Practice, Hazard Assessments, and more. You can still add your own folders alongside them.
Documents now carry an organization document number, a revision, and a status, shown right on each document, so your safety program is properly revision tracked.

Cleaner Needs Your Attention

Changed

The Needs Your Attention grid now hides any tile with nothing in it, so you only see what actually needs action. The grid resizes to fit so there are no empty or orphaned tiles.

QR codes for inspections

Added

Each form now has its own QR code and printable poster, like the safety observation posters. Open a form’s QR page from the QR button on the Inspections & Forms list to view the code, copy the link, or download the poster.
Workers can scan the poster to complete the inspection on their phone. If they are signed in, it submits on their account.
If they are not signed in, they can either log in or continue as a guest by entering their name, with an optional company field, so subcontractors who are not in your system can still complete your inspections. Guest submissions are recorded with that name and company.

Export the training matrix to Excel

Added

An Export Excel button on the training matrix produces a styled, landscape spreadsheet of every worker’s training, one column per course, with the same colour coding as the app: valid, expiring within 30 days, expired, and required-but-missing.
The spreadsheet has filter controls on every column, including a Project column, so you can narrow to a single project right inside Excel. Workers on more than one project appear once per project. Expired, expiring, and missing counts per worker are included for quick sorting.

Time zones, submission filters, consistent branding

Added

A default time zone in Organization settings, used to stamp submitted forms and reports.
A per-project time zone override in project settings, for companies working across time zone boundaries. Projects inherit the organization default unless you set one.
Search and filters on the Submissions page: find forms by text, form type, project, submitter, and date range.

Changed

Submitted form timestamps now display in the project’s time zone and show the zone (for example MDT), on the Submissions list, the detail view, and the PDF.
The safety review report PDF now uses your organization logo with the same clean TruStar Connect wordmark fallback as the other exports.
