Field Level Hazard Assessments and JSAs

Know when to do a field level hazard assessment and when to do it again, fill one in part by part, find today's hazards, rate them on a 5x5 matrix, write controls a crew can follow, break a job into steps for a JSA, and handle sign-on, changes and close-out, in about 55 minutes.

  • Online, on a phone or a computer
  • Professionally narrated
  • Certificate, valid 36 months
Field Level Hazard Assessments and JSAs is for every worker who signs an FLHA, and for the crew leads and supervisors who lead and review them. It shows how the FLHA fits with the formal hazard assessment and the job safety analysis, when an FLHA is done and what calls for doing it again, and the parts of a typical form, from the job and the emergency information to sign-on and review. It covers finding the hazards of today's work by asking what is different today and looking all around, rating each hazard before and after controls on the same 5x5 risk matrix TruStar Connect uses, writing controls that say who, what and where, with PPE always last, breaking a job into steps for a JSA, and stopping to update the FLHA, sign on again and hand over when the job changes. You sort the moments that call for a new FLHA, find answers in a filled-in FLHA for a riser valve change, find six hazards at a wellsite lift, turn vague controls into ones a crew can check, sort a pipe-loading JSA into steps, hazards and controls, make the calls when rain starts, a vacuum truck arrives and a new worker joins, check your understanding after each module, and finish with a 20-question exam. Pass with 80% and your certificate is issued right away, valid for 36 months. Every screen is narrated, with captions and a full transcript. This course provides general training in completing field level hazard assessments and job safety analyses. It does not replace your employer's own forms, procedures and permits, or the orientation and training you need for your site and job.
